When establishing new teams, I wouldn't include dedicated QA roles initially. All members would understand that developers bear quality responsibility. Unit, integration, and UI testing would constitute standard development activities. Continuous integration and test automation would be implemented from inception. Most teams should commence without QA unless specific circumstances dictate otherwise.
